Thank you for choosing Healthy Smiles of Delaware for your oral health care.
In order to serve you and make the most from your time with us, we would like to inform you about your first visit.
When you arrive, we will ask you to fill out a patient questionnaire and medical history, if you have not already done so. These forms are very important; they help to determine the course of your treatment. We have to know about allergies, sensitivity to anesthetics, long-term medications, etc. to get a whole health picture of you.
We will need about an hour or so to examine your teeth, gums, jaw alignment and soft tissue. If you have recent x-rays from your previous dentist, we request that you get a copy of your x-rays and bring them on your first visit; otherwise, we will take the necessary x-rays to find the areas that need treatment.
Healthy Smiles of Delaware, P.A. participates with a number of insurance companies. It is your responsibility to call your insurance company and make the changes necessary to be seen at this office. Please keep in mind that you are responsible for your total financial obligation should your insurance benefits result in less coverage than anticipated. We do ask that you pay your portion at each visit.
